FAQ

CDPAP is a Medicaid-funded home care program, which means that an individual must have Medicaid in order to apply for CDPAP services. Next, the individual needs to be assessed by the New York Independent Assessor [855-222-8350] to prove that they need physical assistance with at least three activities of daily living (ADL) in order to qualify. Activities of daily living include, but are not limited to:

  • Walking
  • Feeding/Eating
  • Dressing
  • Personal hygiene
  • Continence
  • Transportation
  • Shopping
  • Managing finances
  • Meal preparation
  • Housecleaning and home maintenance
  • Managing all forms of communications
  • Managing and taking medications

Pay rate depends on your consumer’s service authorization, and is based on where the Consumer is located. For NYC and the 5 boroughs, base wage would either be twenty dollars and ten cents per hour or twenty-one dollars and seventy-four cents per hour.  For Long Island and Westchester, base wage would either be nineteen dollars and fifty cents per hour or twenty-one dollars and eleven cents per hour.  For the rest of New York State, base wage would either be eighteen dollars and ten cents per hour or nineteen dollars and seventy-one cents per hour.

A Caregiver (Personal Assistant) must be at least 18 years old and eligible to work in the United States, provide pre-employment medical items as required by New York State Department of Health and complete standard application forms. To note, the caregiver cannot be the spouse of the individual receiving care, a parent of a child under age 21, or a designated representative. (A designated representative helps an individual manage money received from NYS SSP.)

Community Care Companions dba. Community Care Home Health Services is an approved CDPAP Facilitator working in partnership with PPL.  Benefits under PPL will include:

Retirement: You will be eligible to enroll in PPL’s 401(k) plan on the first day of the month after you have completed 30 days of employment with PPL.

Professional Development: You will receive up to four professional development hours from PPL. You will be paid for the time spent on your professional development.

Other Benefits: As allowed by applicable law, you will receive Social Security Benefits, New York State Unemployment Compensation, Workers’ Compensation, New York State Disability Insurance, and Paid Family Leave benefits.

Paid Time Off (PTO): You will earn one hour of PTO for every 30 hours worked, up to 56 hours each year. You will be allowed to carry over unused PTO from one year to the next. We will provide you with PPL’s Paid Time Off (PTO) policy for more details once you’re registered.

Health Insurance:

MEC/Flex Plan: After you begin work, you will automatically be enrolled in the PPL MEC/Flex plan. The MEC/Flex plan consists of the PPL Minimum Essentials Coverage, Preventive Plan (MEC Plan), which is a health insurance plan, and the PPL Flexible Benefit Plan (Flex Card), which is a benefits card plan that can help cover the following expenses:

  • Expected benefit HRA (EBHRA): Up to $2,100/year for medical expenses like copays, prescriptions, dental care, and more.
  • Health Reimbursement Arrangement (HRA): No spending limit for dental and vision services.
  • Transit: Up to $315/month for mass-transit passes.
  • Cell Phone: Up to $125/month for work-related cell phone bills.
  • Parking: Up to $315/month for parking expenses.
  • Dependent Care: Up to $5,000/year for eligible dependent care costs.

Voluntary Enhanced Health Insurance Plan: Available to full-time employees, working 130 hours or more per month, only as a replacement of the MEC/Flex Plan, you will be eligible to enroll in PPL’s Minimum Value Plan (MV Plan). This plan provides enhanced health care coverage as compared to the MEC/Flex Plan.

The number of approved hours varies from individual to individual depending on the results from the required New York Independent Assessor review. Individuals may receive just a few hours per week, while others receive 168 hours per week of home care services. There is no average number of hours that an individual can receive.
