CDPAP Program Eligibility Requirements – Consumer
- You must be eligible for New York State Medicaid
- You must live in New York State
- Require assistance with activities of daily living (ADL) or skilled care
- Have a medical condition, be self-directing or have a designated representative that is willing and able to direct care
